I understand that pets sometimes need veterinary care, including visits for common problems such as colds, diarrhea, or parasites, and that all veterinary expenses will be my financial responsibility. I understand that housebreaking, training, and re-training may take lots of time and I am willing to devote that time. I understand that this pet may take several weeks or months to fully adjust to me, my family, and my home. I will take my new pet to a veterinarian soon after adoption for a thorough check-up and appropriate shots. (The MEHREGAN RESCUE GROUP will provide a record of your pet’s medical care to take to your veterinarian) If I adopt an unaltered pet, I will have my pet altered before the spay/neuter certificate expires. I understand that many of these animals are rescue animals and their medical history is unknown. These animals are NOT tested for the disease. The animal’s breed is not definite, and not guaranteed in any way. I will contact the MEHREGAN RESCUE GROUP if I have a problem with my pet. I understand that a home visit is REQUIRED before final placement. I understand that a home visit DOES NOT guarantee placement. I understand that my dog must walk on a leash at all times and that he/she must be walked on a harness, soft fabric slip collar, or choke chain. Walking my dog on an ID collar puts the dog in unnecessary danger and I agree that I will not do so under any circumstance. I understand The MEHREGAN RESCUE GROUP reserves the right to refuse adoption to any applicant for any reason. If approved, this questionnaire becomes part of our contract. I agree that if this animal is not what I’m looking for, or I can no longer have an animal where I reside, I will contact the MEHREGAN RESCUE GROUP immediately for re-adoption. Unfortunately, no refunds will be provided since fees are used for spay/neuter and travel costs. I certify that all statements I made on this adoption application are true and correct. I agree that the MEHREGAN RESCUE GROUP has the right to confiscate a dog placed with me in the event any statements made by me are found to be false. At MEHREGAN RESCUE GROUP, safeguarding your privacy is our utmost priority.
